Service Description
Time to implement your established treatment plan! Follow-up sessions must be booked within 60 days of a Flytta PT Evaluation or previous session for the same condition. New or changed conditions or follow-ups scheduled after 60 days from a previous session will be rescheduled as PT Evaluations. Treatment time will be limited as a result of prioritizing evaluation to get clarity on your current condition and treatment needs. Virtual Visits are hosted by Google Meet. Please make sure you can successfully access this app independently prior to booking. More information will be sent once appointment is booked. Typical Office Hours: 9:30AM-3PM; Early morning appointments available at patient request.
Cancellation Policy
To cancel or reschedule, please contact us no later than 12 hours prior to your appointment time. If your next appointment availability is unknown at the time of reschedule/cancel, a credit with the value of your visit will be applied to your account for future appointments or services. Late Cancellation/Rescheduling: Contacting us less than 12 hours prior to your appointment time may result in a late charge of 50% of your visit rate. Late charges account for the prep work from your provider, administrative time spent to modify schedules, and difficulty to schedule other clients with short notice. For in-person visits, please consider requesting to switch to a virtual visit in the case of weather or unplanned time constraints.
